Position: Home Health Social Services Coordinator – Full-Time Employment Contract
Location: Williamsburg, KY (Whitley County)
Salary: $21 per hour, depending on experience
Schedule: Monday – Friday, 8 am to 4 pm
Minimum Requirements
* Must have a bachelor’s degree in Social Work, Psychology, or Sociology
* Prefer at least one year of experience in professional social work that includes patient assessment and case management;
* Must maintain a valid driver’s license and vehicle insurance.
* Must be organized, confident in skills, able to work independently and prioritize tasks.
* Criminal background check and drug screening required.
Candidates may be excluded for misdemeanor offenses related to abuse, neglect or exploitation or felony convictions.
* Must be up-to-date on COVID-19 vaccinations/booster or willing to be vaccinated within 4 weeks of hire.
Responsibilities:
In the Home Health Program, provides professional social work services through the assessment of client needs and the provision of social services on complex cases and acts as a resource for other staff.
* Conduct a social assessment by interview in home settings to determine services and resources needed
* Prepare treatment plan based on the assessment in conjunction with nursing and other care
* Provide services or assist patient in acquiring services within the community
* Visit homes of patients as indicated to assist with social, emotional, financial and housing issues
* Document in patient records, services received, changes, problems, plans
* Attend meetings and educational activities
* Assist in formulation of policy recommendations
* Initiate and review the preparation of technical, professional and administrative reports
* Coordinates agency functions and resources with other governmental and private agencies
* Plan, prepare and conduct education programs and presentations for staff and community
Work will require travel to family homes or within the community setting when not in the agency office.
Benefits will be provided as defined for full-time employees by 902 KAR Chapter 8 to include retirement, leave, and insurance.
